Sick Tray Request Form
Housing and Residence Life and NMU Dining would like to ensure the wellness of all students using the dining halls by offering the Sick Tray Take-Out Request form on-line for those currently ill with the flu. You may download the Sick Tray Request Take-Out form, and then take the completed form to your Resident Advisor (RA) or Resident Director (RD) for them to sign.
Remember - NMU Dining requires a RA/RD signature on the form and your NMU id in order for a roommate, suitemate, or friend to pick up your meal. Most importantly STAY IN BED AND REST. Do not come into the dining halls if you are sick.
